A completed Parish Verification Form is required for all Catholic families who will be receiving the Catholic Tuition Rate. Please complete the form below and return it to Mrs. Kellyn Vines in the JCCHS Business Office:

Drop-off between 8am-3pm (Monday-Thursday) until the week school starts.
